About The Opportunity

As the Head of Population Health, you’ll play a pivotal role in shaping our value-based care strategy and implementing programs aimed at enhancing clinical outcomes and reducing healthcare costs. Collaborating closely with the Chief Medical Officer, you’ll spearhead initiatives to deepen partnerships with Medicaid Managed Care Organizations, particularly in serving vulnerable populations. Your responsibilities will span from strategy development to program execution, monitoring, evaluation, and continuous improvement, ensuring impactful results.

What You’ll Do

Analyze market dynamics, policy changes, and Medicaid MCO requirements to devise a comprehensive value-based care roadmap.

Collaborate with the CMO to pioneer innovative value-based care models tailored to Medicaid populations.

Lead the end-to-end development and scaling of new health programs in collaboration with clinical, operations, and product teams.

Utilize healthcare quality data and stakeholder feedback to refine and enhance programs post-launch.

Engage with cross-functional teams to identify and capitalize on new value-based care opportunities.

What You’ll Need

5+ years of program or product management experience, with at least 2 years in an early-stage startup environment.

Proven track record of successfully launching clinical products/programs; experience in digital health startups is advantageous.

Ownership mindset with a focus on driving results, business growth, and exceptional customer experiences.

Strong collaboration skills and the ability to work effectively with diverse teams.

Proficiency in quantitative analysis and qualitative research methodologies to inform strategic decisions.

Enthusiasm for working in a fast-paced, startup environment with minimal bureaucratic constraints.

A passion for making a difference in the lives of individuals facing complex chronic needs.
